The Alliance for International Medical Action (ALIMA) is an independent humanitarian medical NGO that was created in 2009 by professionals of humanitarian medicine. ALIMA’s mission is to provide medical care in emergency situations or medical catastrophes.

Job Title: Medical Research Assistant, Acute Malnutrition-Early Warning Score (AM-EWS) Study
Location
: University of Maiduguri Teaching Hospital, Borno
Report to: Research Fellow Hospital for Sick Children, Toronto/Medical Referent of Muna Project, Maiduguri
Research Study

  • The current methods used for detecting high risk children with severe acute malnutrition within the CMAM model Current mortality rate is high (range 5-10%). The purpose of the study is to provide evidence that objective methods for detecting high risk children can be used to optimize efficiency of Community-based Management of Acute Malnutrition (CMAM) treatment programs and thus improve child health outcomes.
  • We will be collecting a set of standard health information (heart rate, breathing rate, level of anemia) from children ill with acute malnutrition admitted to the inpatient therapeutic feeding centre (ITFC) to see if we can use them all together to determine which children are at highest risk of getting sicker and would need to be more closely monitored to better care for them. Vital signs and clinical data will be collected from children admitted into the ITFC twice daily for the duration of their stay in hospital. This study is being conducted collaboratively by Alliance for International Medical Action (ALIMA), The Hospital for Sick Children, and The University of Maiduguri Teaching Hospital (UMTH).

Working Condition:

  • Work environment: work takes place within the inpatient therapeutic feeding centre (ITFC) run by ALIMA in University of Maiduguri Teaching hospital

General Job Description

  • To oversee the study implementation in an ITFC run by ALIMA in The University of Maiduguri Teaching Hospital (UMTH) and ensure transmission of the data through secure internet-scan system to the Hospital for Sick Children. Be the focal person for the study in the ITFC, communicate with ITFC personnel and the UMTH staff pediatricians. Responsible for direct contact with the Research Fellow at Sick kids.

Duties and Responsibilites

  • Facilitate identification of SAM children eligible for inclusion upon ITFC admission
  • Supervise study team including 4 Nurse data collectors and one data archivist).
  • Assure the integrity of the informed consent process
  • Oversee accuracy of data collection by the team
  • Ensure study register is accurately maintained.
  • Support data archivist with transmission of data through secure server
  • Maintain Dinamap (automatic blood pressure) and Rad-G (pulse oximeter) in working condition
  • Maintain regular contact with Sick kids Research Fellow with weekly meetings.

Requirements
Education and Training:

  • Medical Doctor
  • Diploma/ Certificate (or equivalent) in Epidemiology or research related studies

Knowledge and Experience:

  • Fluent in English
  • Experience in research projects
  • Experience in coordination of team
  • Experience with care of children acute malnutrition

Skills and Competencies:

Job Title: Anesthetist
Location:
 Borno
Background

  • ALIMA started SRH activities in Monguno MCH in 2017, and have been referring C-section cases to general hospital Monguno which are performed by the ALIMA OT TEAM and MOH Staff before commencing full CEMONC activities in general hospital Monguno as of April 2019.
  • From September 2017 until the end of June, 160 caesarean section, 5 laparotomies for cesarean complications were performed at Monguno General Hospital
  • The context of Monguno with the security requirements limits references of unstable patients to Maiduguri.
  • However, besides the gynecological obstetric cases that arrive in the structures supported by ALIMA, several other surgical cases requiring local management are also referred by ALIMA to Maiduguri.
  • On average 8 surgical acute abdomens that require a laparotomy, 4 cases of bullet wounds per month require surgery before referencing.
  • The General Hospital of Monguno has an operating room equipped by ALIMA with a well-defined patient circuit and a staff allocated for this activity, including 2 doctors with gyneco obstetric skills and a qualified anesthetist nurse.
  • The mission of the anesthetist will lead to the evaluation of the practice of anesthesia and the safety of patients in the operating room by administering anesthesia and resuscitation of good quality.

Justification for the need of Anesthetist

  • Monguno is the most secure and stable LGA of all LGAs in the north, we note several waves of idps from LGAs around BK attacks on the army and the civilian population.
  • ALIMA is the only medical NGO that rehabilitated the operating room for the activities of CEMONC. To know finance by (DEU), the ministry promises to send the staff with skills surgical for all other activities other than the CEMONC finally to meet the needs of the Monguno population outside cesarean section. Monguno is the most secure and stable LGAs of all LGAs in the north, we note several waves of LGAs around BK attacks on the army and the civilian population.

General Objective

  • The Anesthesiologist will evaluate the needs of anesthesia and resuscitation and then improve the quality of the practice of anesthesia and resuscitation in Monguno.

Specific Objectives:

  • Evaluate the activity of anesthesia and resuscitation, training and coaching:
  • Useful drugs for block, anesthetic products and materials
  • Organization in the operating room (block pharmacy, inventory and order)
  • The practice of anesthesia and monitoring of patients.
  • The implementation of anesthetic and resuscitation protocols.
  • Ensure pre-anesthetic visits and fill out the cards for any patient passing through the block
  • Categorize patients according to the risky anesthetics (ASA1-6)
  • Ensuring the right anesthetic technique with less risk:
  • Spinal anesthesia
  • AG with or without orotracheal intubation
  • Combined anesthesia
  • Ensure the resuscitation training of the newborn and the adult in the other units (IPD, ITFC, SRH.
  • Ensure the sterilization is proceeding in the international and standards norms.
  • Ensure the utilization of antibiotic and analgesic are following standards norms.

Requirements

  • Diploma required
  • Doctor with a Degree in Anesthetist-resuscitator.
  • Nurse specialized in Anesthesia resuscitation with a minimum of 5 years of experience.

Skills:

  • Anticipate and set up an organization of the anesthesia site according to the patient, the type of intervention and the type of anesthesia
  • Respect for the strict rules of health and safety of the patient
  • The anesthetist is partly responsible for the traceability of operating room activities and sterile medical devices as well as the control of disinfection and sterilization procedures.
  • Analyze the situation, anticipate the associated risks according to the type of anesthesia, the patient’s characteristics and the intervention and adjust the anesthetic management
  •  Implement and adapt the anesthesia procedure according to the patient and the course of the procedure
  • Assure and analyze the quality and safety in anesthesia resuscitation
  • Analyze the behavior of the patient and ensure support and information adapted to the situation of anesthesia
  • Coordinate some actions with stakeholders and train professionals in the context of anesthesia-resuscitation, intra and extra-hospital emergency and pain management.
  • Flexibility of adaptation to work with some limited resources.
  • Adhere to the values ​​of ALIMA’s charter.

Language

  • Fluent English.

Job Title: Gynecologist
Location:

Background

  • ALIMA started SRH activities in Monguno MCH in 2017, and have been referring C-section cases to general hospital Monguno which are performed by the ALIMA OT TEAM and MOH Staff before commencing full CEMONC activities in general hospital Monguno as of April 2019.
  • Since 2018, via BEMONC services we have done 928 normal deliveries, 225 complicated deliveries, and 86 C-sections.From January to May 2019, we did a total of 330 deliveries in BEMONC and CEMONC, and we have performed 43 C/sections so far.

Justification for the need of Gynecologist

  • Implementation of training and support based on previous evaluations
  • ALIMA Monguno project seeks the support of an expert obstetrician-gynecologist to assist the SRH activities in the improvement of quality of care, capacity building of the SRH team.
  • The obstetrics and gynecologist expert will give training and coaching to the SRH team in all units (Bemonc and Cemonc) and OT and follow universal standard precautions during and after procedures for effective infection control
  • The Expert will train and mentor the team to reduce maternal morbidity and mortality in Monguno e, which will be achieved through practical training of doctors and other team members in obstetric skills, first aid skills of the newborn for example, surgical management of PPH, etc., obstetric USS, etc.
  • The Expert will also participate in both emergency obstetrical so as to build the competence of the OT team.
  • Because of the high neonatal morbidity and mortality resulting from fetal suffering and prematurity, the expert will help the sexual and reproductive health team to manage these conditions and reduce neonatal morbidity and mortality.

Duration

  • 3 months
  • At the end of visit a report with recommendation will be needed included also the evolution according with the previous evaluation.

General Objective

  • Support (training and coaching) the SRH activities in Monguno to improve quality of care and reduction of maternal and neonatal morbidity and mortality

Specifics Objectives:

  • Capacity building of the OT TEAM to improve surgical management of obstetrics emergencies and improve caesarean section skills in EMONC:
    • Finalize supporting documents on the criteria for identifying high-risk pregnancies, complicated deliveries and gynecological emergencies
    • Reinforcement cesarean section indication
    • Training and support of staff on the resuscitation of the newborn baby
    • Training on maneuverings procedures to facilitate the change of position and delivery of the baby.
  • Build the capacity of the doctors in surgeries, c/section, surgical management of PPH, obstetrics USS etc.
  • Training on how to identify and prevent fistulas and how to support patients before repair.
  • Ensure hands-on training on continue medical education for the SRH team
  • Support the team in the management of obstetrics cases complicated by medical conditions e.g intestinal obstruction in pregnancies etc.
  • Institute contemporary approach in the management of controversial obstetrics complications etc.
  • Recommend need for the procurement of surgical items like doyen’s retractor and other surgical consumables e.g chromic catgut and vicryl absorbable sutures size 2.
  • Provide support to staff in the reference criteria for the admission of newborns between maternity and neonatology
  • Put in place basic SRH protocols and finalize supporting documents on the criteria for identifying high-risk pregnancies, complicated deliveries and gynecological emergencies.
  • Make a report of activities and recommendations on them

Requirements

  • Doctor with a Diploma of Gynecologist-Obstetrician.

Job Title: Laboratory Technician
Location
: Askira-Uba,  Borno
Contract Duration: 6 months
Main Purpose

  • Perform clinical laboratory activities according to ALIMA protocols, security and hygiene measures in order to obtain results to determine the patient’s treatment.

Accountabilities
According to medical prescription, prepare and perform clinical exams/tests with appropriate techniques and following ALIMA protocols and procedures. This function includes, among others:

  • Taking blood, vaginal and urethral samples.
  • Centrifuge blood and urine samples.
  • Perform laboratory exams (serology, hematology, biochemistry, bacteriology or parasitological, according to context of the mission).
  • Ensure proper reporting and registration of the sample results in order to keep a written track of activity and give information to the relevant persons, ensuring confidentiality.
  • Take care, manage and organize efficiently the laboratory material/equipment:
  • Apply hygiene and safety protocols, doing daily cleaning /maintenance, sterilization, washing general glassware after procedures are done.
  • Ensuring proper sample waste disposal according to ALIMA protocols and highest safety standards.
  • Supply orders, do stock, storage conditions follow-up, keep inventory.
  • Keep organized the data collection system of laboratory results and all information generated from the analysis done.
  • Report all relevant information linked to the activity in the laboratory to the line manager, especially the loss/robbery/damage of laboratory equipment or medicines.
  • Ensure that the recipients are transfused with ABO/Rh compatible donor blood
  • Screen donor blood for syphilis, hepB, HepC and HIV, match cross
  • Include protocols for donor selection, pre-transfusion testing monitoring, investigation of transfusion reaction and stringent clerical systems.
  • Donor blood collection packs.
  • Reagents and equipment required for accurate hemoglobin measurement, ABO/Rh blood group, HIV,HepC,HepB and syphilis testing
  • Suitable cold chain for donor blood
  • Train MOH staff on laboratory and blood safety transfusion management.
  • Community mobilization and sensitization for volunteers blood bank
  • Ensure the  lab materials are well working
  • Ensure stationary is available.

General
